Quaternary prevention is the action taken to identify patients at risk of overtreatment, protect them from unnecessary new health actions, and suggest ethically acceptable interventions. The objective of this study is to describe the perceptions of actions that are drivers or barriers for the practice of quaternary prevention, in Primary care doctors of Rebagliati Health network at Social Security in Lima, Peru, on 2018. We carried out a mixed cross sectional study, using the Quantitative-Qualitative methodological triangulation. For the quantitative component, que conducted a survey among all primary care physicians of Rebagliati health Network. We measure the importance they assigned to the practice of quaternary prevention and relevance of Choosing Wisely recommendations (choosing wisely); We used a phenomenological approach in the qualitative component, seeking to reconstruct how the experience has been lived, interpreted by the actors themselves regarding the barriers and facilitators of the practice of quaternary prevention. Results: Most of the interviewees considered the practice of quaternary prevention important (94%) and consider relevant the Choosing Wisely recommendations (59.4%). In the in-depth interviews, the doctors stated that there was a possibility of causing harm with medical practice and identified barriers for quaternary prevention in the health system, the socio-health conditions of the patient and professional burnout; they identified as facilitators of quaternary prevention practice their communication skills and their permanent updating for clinical decision making.La prevención cuaternaria es la acción tomada para identificar a los pacientes en riesgo de sobretratamiento, protegerlos de una innecesaria nueva acción sanitaria y sugerir intervenciones que son éticamente aceptables. El objetivo de este estudio fue describir las percepciones sobre barreras y facilitadores de la práctica de prevención cuaternaria en los médicos que trabajan en los establecimientos de salud de la Red Prestacional Rebagliati del primer nivel de la Seguridad Social en Lima Metropolitana, Perú, en el año 2018. Se realizó un estudio descriptivo mixto, que utilizó la triangulación metodológica Cuantitativa-Cualitativa. Para el componente cuantitativo se realizó una encuesta a todos los médicos establecimientos del primer nivel de atención de la Red Rebagliati para medir qué importancia le asignaban a la práctica de la prevención cuaternaria y la relevancia de las recomendaciones Choosing Wisely (escogiendo sabiamente); en el componente cualitativo se utilizó el enfoque fenomenológico buscando reconstruir cómo se ha vivido la experiencia, interpretada por los propios actores respecto a las barreras y facilitadores de la práctica de la prevención cuaternaria Resultados: La mayor parte de entrevistados consideraban importante la práctica de la prevención cuaternaria (94%) y relevantes las recomendaciones Choosing Wisely (59.4%.). En las entrevistas a profundidad, los médicos manifestaron que existía posibilidad de causar daño con la práctica médica e identificaron barreras para la prevención cuaternaria al sistema sanitario, a las condiciones socio sanitarias del paciente y al desgaste profesional; identificaron como facilitadores sus habilidades de comunicación y a la actualización permanente para la toma de decisiones clínicas.
