Antecedentes: La infección por Helicobacter Pylori (H. pylori) tiene una alta prevalencia en países en vías de desarrollo; en Perú se han reportado prevalencias mayores al 50% en biopsias gástricas de pacientes pediátricos. Es importante conocer el nivel de adherencia al tratamiento en esta población, los efectos adversos más comunes y las causas de baja adherencia con el fin de preverlas, optimizar el tratamiento y disminuir las tasas de resistencia antibiótica. Objetivo: Este estudio tiene como objetivo principal determinar la evidencia existente del nivel de adherencia al tratamiento contra H pylori en pacientes menores de 18 años. Material y métodos: Llevamos a cabo una revisión sistemática siguiendo las recomendaciones (Cochrane). Se realizaron búsquedas en Pubmed, Scielo, Lilacs y Cochrane Library, de 1984 en adelante. Dos revisores seleccionaron de manera independiente los títulos, revisaron los artículos en texto completo, extrajeron los datos y evaluaron el riesgo de sesgo con la Escala Newcastle Ottawa. Resultados: Se incluyeron nueve estudios, ocho ensayos clínicos y una cohorte, se encontró un nivel de adherencia con rango de 69 a 100%. Ocho de los nueve estudios reportaron efectos adversos, siendo el más frecuente las náuseas (6,28%) Conclusiones: La adherencia al tratamiento de H. pylori en población pediátrica en ensayos clínicos es muy alta, los eventos adversos fueron poco frecuentes; se recomienda estudiar la adherencia en condiciones de rutina. Esta es la primera revisión sistemática que evalúa el nivel de adherencia al tratamiento contra H. pylori en población pediátrica.
Background: Helicobacter Pylori (H. pylori) infection has a high prevalence in developing countries; prevalence greater than 50% have been reported in gastric biopsies of peruvian pediatric patients. It is important to know the level of adherence to treatment in this population, the most common adverse effects and the causes of low adherence in order to anticipate them, optimize treatment and reduce the rates of antibiotic resistance. Objective: The main objective of this study is to determine the existing evidence on the level of adherence to treatment against H. pylori in patients under 18 years of age. Methods: We carried out a systematic review following the recommendations (Cochrane). We searched Pubmed, Scielo, Lilacs and Cochrane Library, 1984 onwards. Two review authors independently screened titles, reviewed full-text articles, extracted data, and assessed risk of bias with the Newcastle Ottawa Scale. Results: Nine studies were included, eight clinical trials and a cohort study, the level of adherence found was within 69 to 100%. Eight of the nine studies reported adverse effects, the most frequent being nausea (6.28%). Conclusions: Adherence to H. pylori treatment in the pediatric population in clinical trials is very high; adverse events were infrequent; it is recommended to study adherence under routine conditions. This is the first systematic review to evaluate the level of adherence to H. pylori treatment in the pediatric population.
