La ciudad de Lima es reconocida como una de las ciudades más contaminadas en Latinoamérica por las altas concentraciones de PM2.5, siendo la zona Lima Norte y Lima Este las que superan el Estándar de calidad ambiental reconocido por OMS (25 μg/ m³) y MINAM (50 μg/m³). Este contaminante está asociado con enfermedades respiratorias, cardiovasculares, neurológicas, cáncer, así como efectos adversos en el embarazo y el recién nacido. El objetivo del presente estudio fue determinar la asociación entre la muerte fetal y la exposición a PM2.5 en gestantes residentes de los 43 distritos de Lima Metropolitana, atendidas en el Hospital San Bartolomé y el Hospital Santa Rosa durante el periodo del 2010 al 2016 con la finalidad de determinar diferencias entre Lima Norte y las otras zonas de Lima. La presente investigación es un estudio de ecológico de grupos múltiples con información del Sistema Informático Perinatal de ambos Hospitales, en tanto que las concentraciones diarias de PM2.5, fueron obtenidas de las estimaciones de modelos estadísticos avanzados para los 43 distritos de Lima. La ciudad de Lima fue clasificada en 4 zonas: Lima Norte, Lima Sur, Lima Centro y Lima Este. Mediante un modelo de regresión logística condicional se evaluó la asociación entre muerte fetal y PM2.5, encontrando en el primer y tercer trimestre un OR = 0.97 (IC95% 0.92-1.0) (p>0.05) y en el segundo trimestre un OR = 0.96 (IC95%: 0.92-1.01) (p>0.05), y al incluir otras variables como edad de la madre, control prenatal, anemia, preeclampsia y obesidad tampoco se observaron asociaciones estadísticamente significativas entre muerte fetal y PM2.5. La gestantes de Lima Norte y Este con mayor exposición a PM2.5 no mostraron mayor tasa de muerte fetal (p>0.05). En conclusión, los resultados muestran que no hay asociación entre la exposición a PM2.5 y la muerte fetal en Lima Metropolitana.The city of Lima is recognized as one of the most polluted cities in Latin America due to the high concentrations of PM2.5, being the Lima North and Lima East areas those that exceed the Environmental Quality Standard recognized by WHO (25 μg / m³) and MINAM (50 μg / m³), pollutant associated with respiratory, cardiovascular, neurological diseases, cancer; even adverse effects on pregnancy. The objective of this study was to determine the association between fetal death and exposure to PM2.5 in pregnant women residing in the 43 districts of Metropolitan Lima, treated at the Hospital San Bartolomé and the Hospital Santa Rosa during the period from 2010 to 2016 aimed to demonstrate difference between North Lima with other places of Metropolitan Lima. The present investigation is an ecological study of multiple groups with information from the Perinatal Computer System of both Hospitals, while the daily concentrations of PM2.5 were obtained from the estimates of advanced statistical models for the 43 districts of Lima. The city of Lima was classified in 4 areas: North Lima Norte, South Lima South, Center Lima and East Lima. Using a conditional logistic regression model, the association between fetal death and PM2.5 was evaluated, finding in the first and third trimester an OR = 0.97 (CI95%: 0.92-1.00) (p>0.05) and in the second trimester an OR = 0.96 (CI95%: 0.92-1.01) (p>0.05), and by including other variables such as maternal age, prenatal control, anemia, preeclampsia and obesity, statistically significant associations between stillbirth and PM2.5 were also not observed. Pregnant women from the Lima North and East areas with greater exposure to PM2.5 did not show a higher rate of fetal death (p> 0.05). In conclusion, the results show that there is no association between exposure to PM2.5 and fetal death in Metropolitan Lima.
