Introducción: El síndrome metabólico podría afectar en alcanzar el objetivo de reducción de peso en obesidad. Investigamos si en mujeres con obesidad, el síndrome metabólico influye en la reducción de peso. Materiales y métodos: Estudio de cohorte retrospectivo. El estudio incluyó a todas las mujeres con diagnóstico de obesidad que acudieron al “Programa de Peso Sano” de un hospital general durante el 2009 al 2014. Las participantes fueron agrupadas como mujer obesa metabólicamente sana (OMS) (sin criterios de síndrome metabólico) o mujer obesa metabólicamente patológica (OMP) (con criterios de síndrome metabólico). Se realizaron análisis bivariados entre la reducción de peso y las covariables de interés mediante la prueba de Chi cuadrado o t de Student; y para determinar la asociación entre el fenotipo de obesidad y la reducción de peso, se realizó un análisis de múltiples variables usando la regresión de Poisson con varianza robusta y ajustando por confusores conocidos. Resultados: El estudio incluyó a 239 participantes, el 75.73% fueron catalogadas como mujeres OMP y 24.27% como mujeres OMS. El 57.74% consiguieron el objetivo. Las mujeres OMP tuvieron 23% menor probabilidad de reducir de peso que las mujeres OMS cuando inician un programa de reducción de peso (p=0.004) y esto se mantuvo constante luego de ajustar por edad, grado de obesidad y tipo de tratamiento (p=0.011). Conclusión: Las mujeres OMP tuvieron menor probabilidad de reducir 5% o más de su peso inicial que las mujeres OMS cuando inician un programa de reducción de peso.Introduction: Metabolic syndrome could affect achieving the goal of weight reduction in obesity. We investigated whether metabolic syndrome influences weight loss in obese women. Materials and methods: Retrospective cohort study. The study included all women with a diagnosis of obesity who attended a “Healthy Weight Program” of a general hospital during 2009 to 2014. The participants were grouped as metabolically healthy obese (MHO) women (without metabolic syndrome criteria) or as metabolically pathological obese (MPO) women (without metabolic syndrome criteria). Bivariate analyzes were performed between weight reduction and the covariates of interest using a chi squared test or t-test; and to determine the association between obesity phenotype and weight loss, a multivariate analysis was performed using Poisson regression with robust variance and adjusting for known confounders. Results: The study included 239 participants, 75.73% were classified as MPO women and 24.27% as MHO women. 57.74% achieved the objective. MPO women were 23% less likely to lose weight than MHO women when they started a weight reduction program (p = 0.004) and this remained constant after adjusting for age, degree of obesity and type of treatment (p = 0.011). Conclusion: MPO women were less likely to lose 5% or more of their initial weight than MHO women when starting a weight reduction program.
