Introducción: El cáncer colorrectal (CCR) es el tercer tipo de cáncer en incidencia y el segundo en mortalidad a nivel global, ambas son mayores en hombres. Esta enfermedad se ha tratado por igual en ambos géneros. Sin embargo, múltiples estudios revelan que la prognosis, la farmacología y el riesgo de reincidencia del CCR varían dependiendo del género. Objetivo: Distinguir el perfil metabolómico de hombres y mujeres con pólipos colorrectales y CCR mediante un análisis secundario de datos metabolómicos obtenidos por LC-MS. Métodos: Estudio es del tipo descriptivo y transversal retrospectivo. Resultados: Se obtuvieron un total de 32 y 11 metabolitos diferentes entre hombres y mujeres con pólipos colorrectales y CCR respectivamente. La glicina y valina (p=0.0002 y p=0.0003, respectivamente) mostraron las diferencias más significativas en los pacientes con pólipos y el acetil colina, creatinina y ácido pirotlutámico (p<0.05, p<0.05 y p=0.0001, respectivamente) en los individuos con CCR. Las rutas metabólicas asociadas a los pólipos colorrectales fueron la de síntesis de aminaoacil t-RNA (p<0.05) y la ruta de la glicina, serina y treonina (p=0.003); y al CCR fueron las rutas de la ubiquinona y otros terpenoides (p=0.04) y la de síntesis de vitamina B6 (p=0.04). Conclusiones: Se logró distinguir el perfil metabolómico entre géneros para los tres grupos (sanos, pólipos y CCR) encontrando metabolitos con abundancias significativamente dispares y diferentes rutas metabólicas alteradas por género.Introduction: Colorectal cancer (CRC) is the third type of cancer in incidence and the second in mortality globally, both are higher in men. This disease has been treated equally in both genders. However, multiple studies reveal that the prognosis, pharmacology, and risk of recurrence of CRC vary depending on gender. Objective: To distinguish the metabolomic profile of men and women with colorectal polyps and CRC through a secondary analysis of metabolomic data obtained by LC-MS. Methods: The study is descriptive and retrospective cross-sectional. Results: A total of 32 and 11 different metabolites were obtained between men and women with colorectal polyps and CRC, respectively. Glycine and valine (p = 0.0002 and p = 0.0003, respectively) showed the most significant differences in patients with polyps and acetyl choline, creatinine and pyroglutamic acid (p <0.05, p <0.05 and p = 0.0001, respectively) in individuals with CCR. The metabolic pathways associated with colorectal polyps were the aminaoacil t-RNA synthesis pathway (p <0.05) and the glycine, serine, and threonine pathway (p = 0.003); and to CCR were the routes of ubiquinone and other terpenoids (p = 0.04) and the synthesis of vitamin B6 (p = 0.04). Conclusions: It was possible to distinguish the metabolomic profile between genders for the three groups (healthy, polyps and CRC), finding metabolites with significantly different abundances and different metabolic pathways altered by gender.
