ABSTRACT Cancer is one of the leading causes of death worldwide and nationally. In recent years, the tumor microenvironment (TME) has gained particular relevance due to its role in neoplastic progression, angiogenesis, and immune evasion. Within this ecosystem, tumor-associated macrophages (TAMs) with M2 polarization represent a dominant pro-tumoral component, promoting immunosuppression, invasion, and metastasis. This polarization is dominated by the NP/NK1 axis, and its inhibition is being investigated as a potential therapy in precision medicine. The objective of this scoping review is to map the current evidence on research and treatment focused on this neuroimmunological axis. Results: Nineteen original articles on animal trials and experiments were included, demonstrating the fundamental role of the SP/NK1R axis in the tumor microenvironment and its importance as a therapeutic target. Conclusions: The SP/NK1R neuroimmunological axis is an important M2 polarization pathway; its inhibition with NK1R antagonists has the potential to reverse this phenotype. Clinical evidence is in early stages, and the true effectiveness of the therapies cannot yet be determined. However, numerous trials show high efficacy. It is encouraging that research groups are using it as an adjuvant therapy for cancer, with the future development of new, more effective NK1R inhibitory molecules.
